Intake forms and Liability/waiver forms
As per the new Bill (197) of the Law, every client and patient must sign a waiver before each session to include the scheduled time of service. Also if it's your first appointment, you must complete our, "Getting to Know You" intake forms. If you are not cooperative with this law you will not be able to receive services. All forms and waivers are located here on our website and emailed back (Angelinabacon@yahoo.om) or can be filled out on paper the day of your session. All files are kept safe
Sexual Inappropriateness
*There will be absolutely NO Sexual acts, gestures etc. before, during nor after your session
* Inappropriate touching of the therapist will be grounds for immediate discontinue of your session, the authorities will be called and the full fee will be due for that session. You will never be welcomed to receive services from Gifted Hands Health Center or its professionals ever again
* No sexual conversations will take place
*All therapist will dress in business appropriate attire and will carry themselves as such, not to entice anyone nor spark an inappropriate relationship
*All of the above rules will apply to all therapist here at Gifted Hands, No excuses, hesitations and more
*No extra money will be accepted from clients/patients as favors, promises etc. It will not be tolerated
Liabilities...
* Signing the waiver is very important for every client/patient
* We are not responsible or liable for any massage mishaps if you fail to communicate your full medical issues/conditions and histories as well as any medications currently taken on your intake forms or to your therapist.
*We also are not liable for non communicated information for example allergies to anything to include: oils, lotions, fragrances Etc. being used throughout the session.
* Everyone is reminded to remove all jewelry and valuables from your body. We are not responsible for repairing broken items during the session
Booking & Cancellation Policies... Every booking will be prompted to put any major credit card or debit card on file. This card will only be charged if you cancel within 3-4 hours of your appointment time (which would be 40% of your total fee) or will be used after services are rendered during check out. Denial, Rescheduling or Cancelation of service{s}
*Not obeying all policies set by Gifted Hands' owner or all Florida laws, can cause services to be disrupted, canceled, rescheduled or denied altogether
*(VERY IMPORTANT) If you are ill/sick with a basic cold, Flu, Virus, COVID etc. YOU CANNOT receive a massage unless you/your system is completely clear of symptoms or your doctor gives you the clearance a full week afterwards. You must be out of the contagious stage, being considerate of your therapist, their family and all other patients/clients
*The rule above will also apply to the therapist. If they are sick all sessions will be rescheduled until fully healed of any sicknesses and not in contagious stages
*The therapist will not tolerate rudeness, abuse nor disrespect at any time. This also applies to others in the provided work area/mobile locations
*Your therapist will never be argumentative about fees or the clearing of any medical conditions/sickness from your system, could ruin your chances of receiving services or even get rebooked. All fees are set and will be higher for convenient travel services to your location
*For sanitary purposes, if you do not shower before your massage session or have clean-appropriate feet the therapist will use discretion doing one of the following; deny performing a foot massage, have you to clean your feet first or worse cases, cancel/reschedule your session
Refunds for Services
* Generally we DO NOT give refunds for any service, however if you prepay for a service it will the therapist discretion in working with you, in refunding or rescheduling your session for a better fitting date and time
Mobile/Travel Servicing Policies...
*If someone is ill in your home or office space, your session cannot take place there. You will be asked to reschedule. Please consider the health of your therapist, her family and the other clients/patients that are seen daily.
* While Gifted Hands has the means to travel to your convenient location, we do so only if you are located in the cities/counties north of Boynton Beach. Please call to verify if your location is within our coverage area
* If you are looking for mobile service you must send a message to the therapist after scheduling your session, at the number provided on this website. Stating that you are requesting mobile services
*Fees are higher for mobile services and will be charged based upon the distance of travel/convenience
* If the location you're requesting massage services in, is not in a safe community/location etc., the massage session will automatically be canceled and a cancelation fee will be charged for that appointment
*Upon entering, each location (Residence, office, facility) it must be clean, have a clean scent, and has enough space for the therapist to move freely around the massage table/chair for performing the session
***No children or pets allowed On, Around nor Under the massage table/chair during your relaxation time. It will distract the therapist and they cannot guarantee their work
*Disrespect will not be tolerated neither will a unfit work space/area. Your therapist will leave immediately and the full session/service amount is due before leaving
Gift Certificates...
Currently, Gifted Hands does not offer gift certificates nor gift cards until further notice
